Food Vendors-Tent on Street

This is the Food Vendor Application for prepared food Tent vendors.

This application is for the 2025 Avenues Street Fair on Saturday, September 13th.   

You will serve prepared food or drinks from a canopy or tent on the street.

NOTE: The Avenues Street Fair will not provide power or water to any food vendor. 

This is NOT the application for "pre-packaged" foods.  Use the Retail/Artist application for pre-packaged foods. Check with the County Health Department at Phone Number(385) 468-3845 to see if your goods qualify as "pre-packaged."

You will complete all our application questions.  You will not be charged until acceptance. 

Before you will be accepted, 

A. You will be up to date with the Utah State Tax Commission by reporting sales and paying sales tax from any of your prior events.  Call 801-297-2200 and ask for special events. or visit https://tax.utah.gov/sales/specialevents.

B. You will have the correct food handlers permit. Someone at your booth at all times must have a current food handler's permit.  You will submit a picture to us of this document before your application will be approved. https://www.utfoodhandlers.com/eFoodMainReg

C. You must have a One Day special event license from the Salt Lake County Heath Department.  Call (385) 468-3845, or visit https://www.saltlakecounty.gov/health/special-events/

OR you may have a seasonal special event license.  

You will submit a picture to us of this document before your application will be approved.

C. You will have send us a picture of your tent's fire label which must be CPIA-84 Flame Resistant Material.

D. You will have your own fire extinguisher Type 4A40BC Type K for grease laden vapors, and send us a picture of the current inspection tag.
