Food Vendors - Tent/Canopy

Welcome to the Beaverton Music Fest at Schiffler Park! This is an electronic vendor agreement and by completing this application, you are validating and approving this agreement electronically. 

Please fill out this application completely, missing sections will require us to decline the application. You will have to provide a credit/debit card on the application, however you will not be charged unless accepted as a vendor.  If you would prefer to pay via check, please let us know via email at vendors@frproductionsllc.com.  You will receive an email upon acceptance and will have seven days to send us a check. If you have any questions, please email vendors@frproductionsllc.com

Upon being accepted, you will need to provide General Liability insurance documentation in the amount of $1,000,000 per occurrence/ $2,000,000 aggregate (minimum) naming Florian Raqueño Productions and City of Beaverton, its officers, agents, employees and volunteers as additional insured.  The business name on your insurance MUST match the business name listed on this application. 

No refunds are permitted after acceptance.
