ONLY SUBMIT THIS APPLICATION IF YOU HAVE BEEN CONFIRMED AS A PARTICIPATING VENDOR FOR THE 2025 HISPANIC LATINO HERITAGE MONTH CELEBRATION.

As a participating vendor, you are required to provide a Certificate of Insurance (COI) in order to complete your registration. Please review the attached COI requirements and submit your certificate no later than October 1, 2025 to the Office of Civic Engagement and Social Impact.

If you do not already have your own COI, Eventenly offers the option to purchase one.

Please note: Gwinnett County does not manage or benefit from the Eventenly site. All technical questions or insurance-related issues should be directed to Eventenly.
