KNC 2023 Parental Permission Slip

KuroNeko Cultural Association presents KuroNekoCon: an anime, gaming, and Japanese culture convention based in Spokane, WA. KuroNeko Cultural Association is a nonprofit corporation based in Spokane Washington whose mission is to promote and support community involvement, acceptance of diversity, and understanding of culture through the use of both traditional and contemporary media. Our largest annual event, KuroNekoCon is hosted in the heart of downtown Spokane each summer to take part in a three day community event celebrating Japanese pop culture, individuality, diversity, and artistic and creative expression. The KuroNeko Cultural Association is staffed by a group of dedicated individuals who volunteer their time towards the organization’s mission. All positions are unpaid volunteer positions; 100% of our budget is allotted towards our events and community support.

This is an agreement between KuroNeko Cultural Association and you, the parent or legal guardian of a minor attending KuroNekoCon 2023.

• Attendees who are under the age of 18 must submit this signed permission slip.
• Attendees age 13 to 18 must have the attending adult listed on this form present in the venue at all times.
• Attendees under the age of 13 must be physically accompanied by the attending adult listed on this form at all times.
• If the attending adult is the same person as the legal guardian, enter the legal guardian's name in both sections of the form. The attending adult must be 18 years of age or older and be a registered attendee.

By signing this permission slip you affirm that:

• You have the legal authority to enter into this agreement on behalf of your minor child.

• You give permission for your child to attend KuroNekoCon 2023.

• You agree that KuroNeko Cultural Association staff, organizers, and agents have no responsibility whatsoever to monitor or supervise the minor at any time during KuroNekoCon, and agree to not hold KuroNeko Cultural Association, its staff, or agents responsible for any wrongdoing, claims, demands, or liability.

• In the unfortunate event of damages caused by your minor child at KuroNekoCon, you accept financial responsibility and fully indemnify and hold harmless KuroNeko Cultural Association.

• The attending adult listed below has permission to supervise the minor during KuroNekoCon.
