Thank you for your interest in being a merchandise vendor at our black history program.  This application will be used to gather information about your business and the products you intend to offer at our event.  This application will also serve as an electronic agreement and by electing to be a vendor you are validating and approving this agreement electronically.  

This application is for placement of an advertisement in the event program. The options are a business card size advertisement (2 inches high by 2 inches wide), quarter page (approximately 4 inches high by 2 inches wide),  half page advertisement (approximately 4 inches high by 5 inches wide) or full page advertisement (approximately 8 inches high by 5 inches wide).
